Registered Nurse
Establish, monitor and deliver nursing care to clients within a home setting. Participate in the creation of a nursing plan of care as required and report directly to supervisor. Provide support to unlicensed employees working in the field as required.
Duties and Responsibilities
- Develop and implement a “Plan of “Care for clients in a home setting
- Provide professional support to other staff and employees that is within the scope
of practice of a licensed/registered practical nurse - Adhere to Peak Home Health Care policies and procedures
- Maintain client and corporate confidentiality
- Perform routine nursing duties within scope of practice and according to
Standards of Nursing practices as defined by the provincial licensing body - Observe and report client conditions and changes on an ongoing basis
Education
- Graduate of an accredited licensed/Registered Practical Nursing Program.
- Current registration with provincial licensing body.
Personal Support Worker
Provide personal care services to clients according to an established “Plan of Care”. Such services involve assisting clients with various activities of daily living. All employees must adhere to required internal and external standards of practice. Must have at least one-year experience in personal care.
Duties and Responsibilities
- Provide personal care to clients according to “Plan of Care”
- Report and take instructions directly from supervisor of staff
- Participate in internal training sessions
- Maintain confidentiality of client information
- Document client condition on an ongoing basis and report any risks or concern
to supervisor immediately - Keep and submit records of hours worked
- Adhere to Peak Home Health Care policies and procedures
- Ideally have at least 1 year of direct client/patient care experience in homecare,
community, hospital or long-term care. - Possess valid CPR/First Aid Certificate
- Possess a Cleared Police Background Check for the Vulnerable Sector
- TB Test (2 Step Mantoux) or Chest X-Ray
- Hep B immunization
- Access to car a definite asset
- Flexibility and availability to work overnight or weekend shifts
- Strong Communication skills and ability to read, write and follow verbal or
written instructions - Training and experience with Dementia Care and Alzheimers a definite asset
- Ability to Speak a Second Language an asset
Education
- Graduate of an accredited PSW program or related community service diploma
from a recognized program, or a nursing - Student enrolled in 2nd year of higher in a recognized nursing program (2 years of
placement completed)
Companion
Provide companionship to individuals in a home setting in accordance with an established “Plan of Care”.
Duties and Responsibilities
- Engage in a variety of activities with clients in their home
- Activities include card games, puzzles, reading, walking and other leisure
activities as directed - Report directly to the supervisor of staff
- Keep and submit records of hours worked
- Participate in training programs as required by Peak Home Health Care
- Adhere to Peak Home Health Care policies and procedure
- Maintain client confidentiality
- Notify supervisor of any concerns or health risks that become apparent
Education
- Secondary School Diploma preferred
- One year of experience in companionship duties
